Todd Howard from Bethesda, who made Fallout 3 and Skyrim, gave the keynote speech at DICE this year. It's pretty informative about their team's process. At about the 20minute mark you can see all the fun stuff the developers did when given a free week to do whatever they wanted with Skyrim. None of this stuff is in the game yet, or may never be.
